- Why you believe the individual you are nominating deserves the title SoR UK Radiography Professional of the Year 2023, including:
- A brief description of the individual's career or current role
- Why you are nominating this person for this award
- How this person has made a positive impact on patient experience
- Specific examples or scenarios of when they went above and beyond
- Behaviours, skills or traits that are worthy of recognition
- Your full contact details (including your membership number)

- Why you believe the student you are nominating deserves the title SoR UK Student Radiographer of the Year 2023, including:
- A brief description of their academic achievements or why they decided to study radiography
- Why you are nominating this person for this award
- How this person has made a positive impact on others
- Specific examples of when they went above and beyond
- Behaviours, skills or traits that are worthy of recognition
