The latest Safer Radiotherapy newsletter has been published, outlining the key messages and trends from the analysis of radiotherapy error (RTE) reports from April to July 2019.
Minor incidents made up 98.1% of RTE during the timeframe, with the most frequently reported RTE process code in the near miss classification being ‘use of on-set imaging’.
In addition to the RTE data analysis, the newsletter offers links to patient safety resources, information surrounding the NHS patient safety strategy and important dates for the diary.
A case study looking at the verification of diagnosis/extent/stage has also been published in this edition, which considers a palliative radiotherapy patient having treatment on the brain and upper c-spine.
